Job summary

Matrix Medical Network seeks Nurse Practitioners to perform Comprehensive Health Assessments in members' homes. You will evaluate health status, medications, and risks, documenting findings to improve care quality.

Qualifications

• Must hold NP licensure in the state of practice and prescriptive authority.
• Current BLS/ACLS certification.
• 1 year of NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric health assessments in the member's home.
• Review medical history, medications, and risk factors; document per Matrix guidelines.
• Coordinate with physicians, case managers, and caregivers as appropriate.
• Provide education and follow-up in collaboration with PCP.
• Travel to member residences including homes or SNFs as needed.
