Matrix Medical Network invites experienced nurse practitioners to provide comprehensive in-home health assessments across the nation. You will review health history, med adherence, and social factors to identify gaps in care, with a focus on improving outcomes and coordinating with PCPs.

Opportunities are full-time with flexible scheduling, including weekends. A salary not specified; compensation is per assessment with potential monthly earnings based on volume.

Qualifications

• Master’s Degree or commensurate experience and NP licensure
• Current NP licensure in state of practice to include prescription authority or ability to obtain prescriptive authority as needed
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
• Current BLS, ACLS or CPR certification
• 1 year of NP experience preferred, new grads encouraged to apply
• Valid state driver’s license for the state of residence
• Automobile insurance coverage required for the state of residence
• Strong computer skills and familiarity with employee health/medical record software
• Excellent verbal and written communication skills

Responsabilités

• Conduct adult / geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation according to Matrix guidelines
• Administer and order point of care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers, family members, caregivers, and ancillary staff as appropriate
• Coordinate with Primary Care Physician on patient education and follow up
• Provide services at the member’s residence, which may be in homes or Skilled Nursing Facilities
