Job summary

Matrix Medical Network offers in-home comprehensive health assessments performed by Nurse Practitioners. Providers review medical history, medications, and social factors during visits at the member's residence or Skilled Nursing Facilities, with a focus on closing care gaps and improving outcomes.

Qualifications

NP licensure in the state of practice with prescriptive authority.
Board certification in Adult, Gerontology, Family, or Emergency.
BLS/ACLS or CPR certification.
1 year NP experience preferred; new grads welcome.
Valid driver’s license and auto insurance for residence.
Strong computer skills and familiarity with health record software.

Responsibilities

Perform comprehensive adult/geriatric assessments with medical history and follow-up.
Administer point-of-care and lab-based testing as needed.
Collaborate with PCP, case managers, social workers, and family caregivers.
Educate patients and coordinate follow-up care.
Provide services at member homes or SNFs.
