Job summary

Matrix Medical Network seeks experienced nurse practitioners to perform comprehensive in-home health assessments. Visits last up to one hour and focus on health status, history, adherence, and risk factors, enabling targeted care planning.

Providers work across the United States, with flexible scheduling including weekends, and opportunities for full-time roles with sign-on bonuses. Competitive per-assessment pay and ongoing professional support are offered.

Qualifications

• Must hold NP licensure in the practicing state and be eligible for prescriptive authority.
• Board certification in an adult/gerontology or family NP specialty required.
• Current BLS/ACLS CPR certification and strong computer skills.

Responsibilities

• Perform adult and geriatric health assessments in the member’s residence.
• Review medical history, medications, and social determinants; document per Matrix guidelines.
• Coordinate with physicians, case managers, and caregivers; provide follow-up as needed.
• Administer point-of-care and lab testing; ensure accurate records.
