Job summary

Matrix Medical Network is hiring an NP to perform comprehensive in-home health assessments, reviewing medical history, medications and risk factors. You will collaborate with physicians, case managers and caregivers to close care gaps and ensure high-quality outcomes.

This role offers flexible scheduling, with visits nationwide, and opportunities for licensure support, professional development and a FT pathway in home-based care settings.

Qualifications

• Master’s Degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Gerontology, Family or Emergency NP specialties.
• BLS/ACLS or CPR certification required.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and automobile insurance.
• Strong computer skills and familiarity with health records software.

Responsibilities

• Conduct adult/geriatric health assessments and document findings per Matrix guidelines.
• Order and administer point-of-care and lab tests as needed.
• Collaborate with PCPs, case managers, social workers and caregivers for care plans.
• Provide health education, referrals and follow-up in home or SNF settings.
