Job summary

Matrix Medical Network seeks Nurse Practitioners to perform comprehensive in-home health assessments, reviewing medical history, medications, and risk factors. You will order tests, coordinate with physicians and care teams, and document findings thoroughly.

Work in members’ homes or SNFs with flexible scheduling and potential FT opportunities. Our providers earn based on completed assessments, with support for licensure expansion and ongoing education.

Qualifications

• Master’s degree and NP licensure.
• Current NP licensure in state of practice.
• Board certification in Adult, Gerontology, Family or Acute Care specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license.
• Auto insurance coverage as required by state.

Responsibilities

• Conduct adult/geriatric assessments per Matrix guidelines.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, social workers and care team.
• Provide services at member homes or Skilled Nursing Facilities.
• Document clearly and maintain scheduling protocols.
