Job summary
Matrix Medical Network is seeking a Nurse Practitioner to conduct in-home health assessments, reviewing medical history, medications, and risk factors to close care gaps. You will work with physicians, case managers, and care teams to ensure accurate documentation and appropriate referrals.

Qualifications
- Master’s Degree and NP licensure in the state of practice.
- Current NP licensure with prescriptive authority or the ability to obtain.
- Board certification in adult or family-focused specialties.
- BLS/ACLS or CPR certification.
- 1 year NP experience preferred; new grads encouraged.
- Valid state driver’s license and auto-insurance coverage.
- Strong computer skills and EMR familiarity.
Responsibilities
- Conduct adult/geriatric health assessments and gather medical history.
- Document findings clearly and follow Matrix protocols.
- Administer point-of-care and lab testing as needed.
- Collaborate with physicians, case managers, and care teams.
- Provide in-home health assessments at members’ residences.
