Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments, including medical histories and medication reviews, to close care gaps. You’ll work with PCPs, case managers, and family members, traveling to members’ homes or SNFs.

The role emphasizes collaboration, accurate documentation, and delivering high-quality, personalized care with supportive onboarding and ongoing education.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Family, or gerontology NP specialties (AANP/AACN/ANCC).
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Perform adult/geriatric assessments including medical history, diagnoses, treatments, and care gaps.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers, social workers, family members and other health professionals.
• Provide in-home services at the member’s residence or SNFs.
• Document findings and follow Matrix guidelines and protocols.
