Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform comprehensive in-home health assessments. You will review medical history, medications and social factors, and coordinate with primary care physicians and care teams to close care gaps.

Visits occur in members’ homes or SNFs, with education and follow-up emphasized. Position offers flexible scheduling, potential transition to FT roles, and benefits including licensure support and education resources.

Qualifications

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure
• Current NP licensure in state of practice to include prescription authority or ability to obtain prescriptive authority as needed
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
• Current BLS, ACLS or CPR certification
• 1 year NP experience preferred; new grads encouraged to apply
• Valid state driver’s license for the state of residence
• Automobile insurance coverage for the state of residence (or ability to obtain)
• Strong computer skills and familiarity with employee health/medical record software
• Excellent verbal and written communication skills

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation
• Administer and order point of care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers, family members and other personnel
• Coordinate with PCP on patient education and follow up
• Provide services at member residences (homes or Skilled Nursing Facilities)
