Job summary

Matrix Medical Network seeks Nurse Practitioners to perform Comprehensive Health Assessments in members’ homes or Skilled Nursing Facilities. Visits last up to one hour and cover health history, medications, risks, and care gaps, with documentation to Matrix guidelines.

You’ll order tests, collaborate with PCPs and care teams, and provide education and follow-up. Opportunities include full-time roles with supportive onboarding and licensure assistance.

Qualifications

• Master’s Degree or commensurate experience and NP licensure
• Current NP licensure in state of practice
• AANP, AACN, or ANCC board certification in Adult or Gerontology
• Current BLS/ACLS/CPR certification
• 1 year NP experience preferred, new grads encouraged to apply
• Valid state driver’s license
• Automobile insurance coverage for state of residence

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, and documentation
• Administer and order point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers, and caregivers
• Coordinate patient education and follow-up with PCP
• Provide services at the member’s residence (home or SNF)
