Job summary

Matrix Medical Network seeks a qualified NP to perform Comprehensive Health Assessments in patients' homes or Skilled Nursing Facilities. Each visit lasts about an hour and centers on health history, meds, and risk factors, with documentation per Matrix guidelines.

You'll work with PCPs and care teams, order tests, provide education and timely referrals, and travel to member homes as needed. This FT role offers flexible scheduling and benefits.

Qualifications

• NP licensed in the practice state with prescribing authority.
• Board-certified in Adult/Gerontology/Family or related specialty.
• Current BLS/ACLS/CPR certification and strong documentation skills.

Responsibilities

• Conduct adult/geriatric assessments including history, diagnosis, education and referrals.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers and care teams to coordinate care.
• Provide services at member homes or SNFs with clear documentation.
