Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments that review health history, medications, and risk factors. Visits may occur at the member’s home or skilled nursing facility, with coordination across physicians and care teams to close gaps in care.

The position emphasizes patient education, follow-up, and documentation in Matrix guidelines, supported by onboarding and ongoing education.

Qualifications

• Master’s Degree in Nursing or commensurate clinical experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• AANP/AACN/ANCC board certification in Adult, Gerontology, Family or related specialty.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license for the state of residence.
• Automobile insurance coverage required for the state of residence.
• Strong computer skills and familiarity with health record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and other care team members.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence (home or Skilled Nursing Facility).
