Job summary

Matrix Medical Network is seeking a licensed NP to perform Comprehensive Health Assessments in members' homes nationwide. Each visit lasts up to one hour and focuses on health history, current status, medications, and risk factors.

You’ll order tests, coordinate with physicians, and provide clear documentation and follow-up. This role offers flexible scheduling to fit your availability and opportunities for growth.

Qualifications

• NP licensure in state of practice with prescriptive authority or ability to obtain
• Board certification in Adult, Gerontology, Family or Emergency specialties
• BLS/ACLS certification
• 1 year NP experience preferred (new grads welcome)
• Valid state driver's license and auto insurance
• Strong computer skills and familiarity with medical record software
• Excellent verbal and written communication skills

Responsibilities

• Conduct adult/geriatric health assessments including history, diagnoses, treatment and referrals
• Administer or order point-of-care and lab-based testing as needed
• Collaborate with PCPs, case managers, social workers, and family members
• Provide in-home services at member residences or in SNFs
• Educate patients and provide follow-up care and documentation
