Job summary

Matrix Medical Network is recruiting Nurse Practitioners to perform comprehensive in-home health assessments for members across the United States. Each visit lasts up to one hour and includes review of medical history, medications, and social factors to close care gaps.

Responsibilities include conducting adult-geriatric assessments, ordering tests as needed, collaborating with PCPs, and delivering reports per Matrix guidelines. Full-time opportunities with flexible scheduling are available.

Qualifications

• Master’s Degree in Nursing or commensurate NP experience.
• Current NP licensure in state of practice with prescriptive authority if required.
• Board certification in Adult/Family/Gerontology specialties.
• BLS/ACLS certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric health assessments in patients' homes.
• Review medical history, medications, and social determinants of health.
• Order point-of-care tests and lab-based testing as needed.
