Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform Comprehensive Health Assessments at members' homes nationwide. Each visit lasts up to one hour and includes a review of health status, medical history, medication adherence, and risk evaluation.

Providers collaborate with PCPs, case managers, and care teams, ensuring quality care with flexible scheduling, mileage reimbursement, and opportunities for Full-time advancement. Active NP licensure and board certification required.

Qualifications

• Master’s Degree in Nursing or commensurate experience.
• Current NP licensure in state of practice.
• AANP, AACN, or ANCC board certification.
• BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license.
• Auto insurance for residence state.
• Strong computer skills and familiarity with EMR software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers as appropriate.
• Coordinate with PCP on patient education and follow-up.
