Job summary

Matrix Medical Network, a national in-home clinical network, seeks NP clinicians to perform Comprehensive Health Assessments in members' homes or Skilled Nursing Facilities. Each visit lasts up to an hour to review health history, medications, and risk factors, enabling better care gaps closure.

Providers collaborate with PCPs and care teams, with base salary plus incentive opportunities, travel up to 15%, and support for licensure and continuing education.

Qualifications

• Master’s Degree in NP or equivalent and state licensure.
• Current NP licensure in state of practice with prescriptive authority.
• AANP/AACN/ANCC board certification in applicable NP specialties.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Able to travel up to 15% including overnight travel.
• Valid state driver’s license and auto insurance.
• Strong computer skills and familiarity with EMR software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, family members, caregivers and ancillary staff as appropriate.
• Coordinate with PCP on patient education and follow-up as necessary.
