Job summary

Matrix Medical Network seeks experienced nurse practitioners to perform Comprehensive Health Assessments in members' homes nationwide. Each visit lasts up to one hour and involves reviewing health history, medication adherence, and risks to identify care gaps.

Providers work with physicians and care teams, enjoy flexible scheduling, competitive pay (monthly earnings from $1,800 to $11,500), and benefits including mileage reimbursement and 401(k).

Qualifications

• Must hold an NP license in the practice state.
• Current board certification in relevant NP specialty.
• License to prescribe (prescriptive authority) as needed.
• BLS/ACLS or CPR certification.
• Approximately 1 year NP experience preferred.
• Valid state driver’s license and auto insurance coverage.
• Proficient computer skills and EMR software familiarity.
• Strong verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history and diagnosis.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and family members.
• Coordinate with Primary Care Physician on education and follow-up.
• Provide services at the member’s residence (home or Skilled Nursing Facility).
