Job summary

Matrix Medical Network is seeking a NP to perform Comprehensive Health Assessments in members’ homes, reviewing health history, medications, and social factors to close care gaps. Visits last up to one hour, with services at home or Skilled Nursing Facilities as needed.

The role offers flexible scheduling, competitive pay based on visits completed, and potential transition to full-time roles with additional licensure support.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure with prescriptive authority or ability to obtain.
• Board certification in Adult/Primary Care or related specialties.

Responsibilities

• Perform adult/geriatric assessments including history, diagnosis, education, and referrals.
• Order and administer point-of-care and lab testing as needed.
• Collaborate with PCPs, care managers, and caregivers; document findings per guidelines.
• Provide in-home services at the member’s residence or SNF as required.
