Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform Comprehensive Health Assessments in members' homes across the nation. You will review medical history, medications, and social factors to close care gaps during each one-hour visit.

The role requires NP licensure, board certification in an adult/gerontology or family practice, and the ability to travel up to 80–90% with overnight stays as needed. A strong emphasis on collaboration and patient education is essential.

Qualifications

• Master’s Degree in Nursing and NP licensure required.
• AANP/AACN/ANCC board certification in applicable NP specialty.
• Current BLS/ACLS certification.
• 1 year NP experience.
• Willingness to travel up to 80-90% and occasional overnight travel.

Responsibilities

• Perform adult/geriatric health assessments in member homes.
• Administer point-of-care and lab tests as needed.
• Coordinate with physicians, case managers, and other health professionals.
• Provide patient education and follow-up in collaboration with PCP.
• Travel to members' residences, including SNFs, to complete CHA visits.
