Matrix Medical Network is seeking Nurse Practitioners to perform Comprehensive Health Assessments in patients’ homes or facilities. You will review medical history, meds, and risk factors, delivering care with a focus on quality and coordination with the care team.

These roles offer flexible scheduling, competitive pay based on assessments completed, and opportunities to travel/expand licensure. FT opportunities include sign-on bonuses and long-term career growth.

Qualificações

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience.
• Able to travel up to 80-90% of the time for extended periods, including overnight travel and weekends across the assigned state and possibly neighboring states.
• Valid state driver's license for the state of residence and automobile insurance coverage.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsabilidades

• Conduct adult / geriatric health assessments, including medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other care team members.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence, including homes or Skilled Nursing Facilities.
