Job summary

Matrix Medical Network is seeking experienced Nurse Practitioners to perform Comprehensive Health Assessments in members' homes or Skilled Nursing Facilities. You will review health history, medications, and risk factors, and coordinate care with PCPs and case managers. Each visit lasts up to one hour and may involve testing and education.

The role offers flexibility: visits scheduled around your availability, with nationwide opportunities and potential for a full-time position and sign-on bonus.

Qualifications

• Master’s degree or commensurate experience and NP licensure
• Current NP licensure in state with prescriptive authority or ability to obtain
• Board certification in Adult/AGPC/Nurse Practitioner specialties
• Current BLS/ACLS/CPR certification
• 1 year NP experience
• Able to travel 80–90% of the time including overnight travel
• Valid driver’s license in state of residence
• Auto insurance coverage for state of residence

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, education, referrals and documentation
• Administer and order point-of-care and lab testing as needed
• Collaborate with physicians, case managers, social workers, family members and other staff
• Coordinate with PCP on education and follow-up
• Provide services at member residences (homes or SNFs)
