Job summary

Matrix Medical Network offers in-home comprehensive health assessments conducted by NPs, with schedule flexibility across weekends and weekdays. Providers review health history, medications, and risks to close care gaps in a single visit.

Eligible NPs hold state licensure, board certification, and CPR/BLS; compensation reflects assessments completed, plus potential travel reimbursements and benefits.

Qualifications

Master’s NP degree or commensurate experience with licensure.
Current NP licensure in state with prescription authority.
Board certification in Adult, Gerontology, Family or Emergency NP specialties.
BLS/ACLS/CPR certification.
1 year NP experience preferred; new grads welcome.

Responsibilities

Conduct adult/geriatric assessments including medical history, diagnoses and treatment planning.
Perform point-of-care and lab-based testing as needed.
Collaborate with physicians, case managers, social workers, and caregivers.
Coordinate with PCP for education and follow-up.
Provide services at members' residences or in SNFs.
