Job summary

Matrix Medical Network offers in-home comprehensive health assessments led by a Nurse Practitioner. Providers review medical histories, medications, and home environments to identify risk factors and close care gaps with a single visit.

Visits occur at patients’ homes or Skilled Nursing Facilities, with flexible scheduling across days and weekends. Competitive pay reflects volume of assessments, supportive onboarding, and opportunities for licensure expansion.

Qualifications

Master’s degree in nursing or commensurate experience.
Current NP licensure in state of practice with prescriptive authority.
Board certification in Adult, Gerontology, Family or Emergency NP specialization.
Current BLS/ACLS or CPR certification.
1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

Conduct adult/geriatric assessments including medical history, diagnosis, treatment, and health education.
Administer and order point-of-care and lab testing as needed.
Collaborate with PCPs, case managers, social workers and family members.
Provide home-based health assessments at patients' residences or facilities.
Document findings and coordinate follow-up care per Matrix guidelines.
