Job summary

Matrix Medical Network is expanding its in-home health assessment team. As a Nurse Practitioner, you will conduct comprehensive evaluations at patients’ homes, review medical history, medications, and social factors, and coordinate with PCPs to close care gaps.

Qualifications

• NP licensure in practice state with prescriptive authority
• Board certification in Adult, Family, or Gerontology specialties
• Active BLS/ACLS certification and CPR proficiency

Responsibilities

• Conduct adult/geriatric health assessments in patients' homes.
• Review medical history, medications, and social determinants of health.
• Coordinate with PCPs, case managers and caregivers to close care gaps.
