Job summary

Matrix Medical Network conducts Comprehensive Health Assessments in patients’ homes or Skilled Nursing Facilities, providing in-depth health reviews and risk assessment. Providers review medical history, medications, and social factors to improve care gaps with tailored plans.

This role emphasizes autonomous home visits, collaboration with care teams, and ongoing education and licensure support, with flexible scheduling and competitive pay based on completed assessments.

Qualifications

• Master’s degree and NP licensure in the state of practice.
• Active NP licensure with prescribing authority if required by state.
• Board certification in Adult, Gerontology or Family NP specialties.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid driver’s license and auto insurance where required.
• Strong computer skills and familiarity with health record software.
• Excellent verbal and written communication skills.

Responsibilities

• Perform adult/geriatric assessments including history, diagnosis, treatment and education.
• Order and administer point-of-care and lab tests as needed.
• Collaborate with physicians, case managers and care teams.
• Coordinate patient education and follow-up with PCPs as appropriate.
• Provide services at the patient’s home or in skilled facilities.
