Job summary

Matrix Medical Network offers in-home Comprehensive Health Assessments conducted by NPs to review medical history, medications, and social factors. Visits occur at the member’s residence, with coordination among physicians, case managers, and family caregivers to improve health outcomes.

Providers administer tests, document findings, and collaborate with PCPs for follow-up. Flexible scheduling supports a work‑life fit while delivering high‑quality care in the community.

Qualifications

• Master’s degree or commensurate experience with NP licensure.
• Current NP licensure and prescriptive authority as needed.
• BLS/ACLS or CPR certification is required.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric health assessments and document findings according to Matrix guidelines.
• Administer point-of-care and lab testing as needed.
• Collaborate with physicians, case managers and PCPs to coordinate care.
• Provide services at members’ residences, including homes or SNFs.
