Matrix Medical Network is seeking qualified Nurse Practitioners to perform comprehensive in-home health assessments. Each visit lasts up to one hour and evaluates medical history, adherence, social factors, and risks to identify care gaps.

We offer flexible scheduling and competitive pay, with monthly earnings based on visits. Benefits include mileage reimbursement, 401(k), and ongoing education support, plus opportunities to transition to full-time roles and licensure assistance across states.

Formación

• Master’s Degree in NP or equivalent and active licensure in practice state.
• Current NP licensure with prescriptive authority or ability to obtain.
• Board certification in Adult/AGPC, Family, Emergency or acute care.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid driver’s license and auto insurance for residence state.
• Strong computer skills and familiarity with medical/employee health software.
• Excellent verbal and written communication skills.

Responsabilidades

• Conduct adult and geriatric health assessments including medical history, diagnoses, treatments, and follow-up.
• Order and administer point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Coordinate with PCP for patient education and follow-up.
• Provide in-home services at patients' residences or facilities.
