Job summary

Matrix Medical Network delivers in-home health assessments across the nation, reviewing health history, adherence, and risk factors to close care gaps during each visit. Providers work with physicians, nurses, and care teams to educate patients, order tests, and document outcomes.

Qualifications

• Master's degree or commensurate experience and satisfactory NP licensure.
• Current NP licensure in state of practice with prescribing authority.
• Board certification in Adult/Adult Gerontology/Family/ACNP/EM/Acute Care.
• Current BLS/ACLS CPR certification.
• 1 year NP experience preferred; new grads welcome.

Responsibilities

• Conduct adult/geriatric assessments including medical history and diagnosis.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, and care team to coordinate care.
• Provide in-home assessments at member residences or SNFs.
• Document findings clearly per Matrix guidelines.
