Job summary

Matrix Medical Network seeks NP clinicians to perform comprehensive in‑home health assessments. You’ll evaluate medical history, current health, adherence, and social factors, delivering insights to close care gaps during a single visit.

Providers conduct adult/geriatric assessments, order tests, and collaborate with PCPs, case managers, and families. Services occur at members’ homes or SNFs, with flexible Sunday–Saturday scheduling.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice to include prescriptive authority.
• Board certification in Adult, Gerontology or Family NP specialties.
• Current BLS/ACLS certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license.
• Auto insurance coverage requiring for state of residence.

Responsibilities

• Conduct adult/geriatric assessments including history, diagnosis and education.
• Administer point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and family members.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence (home or SNF).
