Job summary

Matrix Medical Network offers in-home Comprehensive Health Assessments conducted by experienced nurse practitioners. Each visit lasts up to one hour to review medical history, current medications, and risk factors, enabling identification of chronic conditions and care gaps.

Providers work at members' homes or Skilled Nursing Facilities with flexible scheduling (Sunday–Saturday 8 am–7 pm) and competitive pay per assessment.

Qualifications

• NP licensure in the practice state is required
• Board certification in adult/gerontology/family/emergency nursing is required
• Active CPR/BLS or ACLS certification required
• Valid driver’s license and auto insurance coverage required
• Experience preferred; new grads encouraged to apply
• Strong computer skills and experience with medical record software preferred

Responsibilities

• Conduct adult/geriatric assessments including medical history and treatment
• Administer and order point-of-care and lab testing as needed
• Collaborate with physicians, case managers, social workers and caregivers
• Provide education and follow-up with patients and PCPs
• Perform services at members' homes or skilled nursing facilities

Skills

Excellent verbal and written comms

Strong computer skills

Education

Master’s Degree or commensurate NP licensure

NP licensure in state with prescriptive authority

AANP/AACN/ANCC board certification in applicable NP field

BLS/ACLS/CPR certification

Valid state driver’s license
