Matrix Medical Network seeks qualified Nurse Practitioners to perform comprehensive in-home health assessments for adults and seniors. You will review medical history, medications, and risk factors, and coordinate with PCPs for follow‑up plans.

Providers conduct visits at members’ residences, accommodating diverse schedules, with competitive pay and favorable career support including onboarding and continuing education opportunities.

Qualifications

• Master’s degree in Nursing or commensurate experience with NP licensure.
• Current NP licensure in state of practice to include prescriptive authority or ability to obtain.
• Active board certification in relevant NP specialty is preferred.
• Strong computer literacy and familiarity with medical records software.

Responsabilités

• Perform adult/geriatric in-home assessments including medical history and current health status.
• Administer point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate patient education and follow-up with PCPs.
