Job summary

Matrix Medical Network offers in-home comprehensive health assessments led by highly qualified Nurse Practitioners. Providers review medical history, medications, and social factors to identify care gaps and improve outcomes.

Assessments leverage multidisciplinary collaboration and are conducted at members' homes or Skilled Nursing Facilities. Flexibility in scheduling with full-time opportunities, competitive pay based on visits completed, and benefits such as mileage reimbursement and 401(k)

Qualifications

• Master’s Degree in Nursing or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• Board certification in Adult, Gerontology, Family, Emergency or Acute Care.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome.

Responsibilities

• Perform adult/geriatric assessments including medical history, diagnoses and treatment.
• Administer or order point-of-care and lab-based testing as needed.
• Collaborate with PCP, case managers, social workers, and families as appropriate.
• Provide services at member residences (homes or SNFs).
• Document findings and referrals per Matrix guidelines and protocols.
