Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform in-home Comprehensive Health Assessments, reviewing health history, medications, and social factors during visits lasting up to one hour.

You will collaborate with PCPs, case managers, and other clinicians, traveling across the state (up to 80–90%) to support member care and quality outcomes. Opportunities include full-time roles with a signing bonus and competitive pay.

Qualifications

• NP licensure in state of practice
• 1 year of NP experience
• AANP, AACN, or ANCC board certification in relevant specialties
• Current BLS, ACLS or CPR certification
• Ability to travel up to 80-90% including overnight travel
• Valid state driver’s license
• Automobile insurance coverage in home state ($100k/$300k Bodily Injury, $50k Property Damage)
• Strong computer skills and familiarity with health records software

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, and treatment
• Administer and order point-of-care and lab testing as needed
• Collaborate with PCPs, case managers, social workers, and caregivers
• Provide services at the member’s residence (home or Skilled Nursing Facility)
• Coordinate education and follow-up with the Primary Care Physician
• Maintain documentation according to Matrix guidelines and protocols
