Job summary

Matrix Medical Network is seeking qualified Nurse Practitioners to perform comprehensive in-home health assessments, reviewing medical history, medications, social factors, and risks during visits lasting up to one hour. You will help close care gaps by coordinating with PCPs, case managers, and family members, and deliver care in patients' homes or Skilled Nursing Facilities.

We offer flexible scheduling, including weekends, with pay tied to assessments ($1,800–$11,500 per month).

Qualifications

• Must hold NP licensure in practicing state and eligible for prescriptive authority.
• Board certification in Adult, Gerontology, Family, Emergency, or Acute Care.
• Current BLS, ACLS or CPR certification.
• Strong computer skills and experience with EMR systems.
• Valid driver’s license and auto insurance coverage for the resident state.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, and treatment.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, social workers, and family members as appropriate.
• Provide in-home health assessments at the member’s residence or SNFs.
• Document findings per Matrix guidelines and ensure clear documentation.
