Job summary

Matrix Medical Network is expanding its in-home health assessment program and seeks a qualified NP to perform comprehensive assessments at members’ residences. This role emphasizes geriatric care, chronic condition identification, and coordinated care planning with PCPs, nurses, and support teams.

Providers enjoy flexible scheduling, the opportunity to work across diverse settings, and a focus on improving outcomes while delivering compassionate care in patients' homes.

Qualifications

Master’s degree and NP licensure in state of practice.
Board certification in applicable NP specialties.
BLS/ACLS/CPR certification.
Valid driver’s license and auto insurance.
Strong communication skills.

Responsibilities

Conduct adult/geriatric assessments and document per Matrix guidelines.
Administer point-of-care and lab testing as needed.
Collaborate with PCPs, case managers, social workers, and family members.
Provide services at member residences including homes or SNFs.
