Matrix Medical Network delivers in-home Comprehensive Health Assessments across the United States, reviewing medical history, medications, and risk factors during visits that can last up to an hour.

As a Nurse Practitioner with state licensure, you will perform tests, coordinate with PCPs and care teams, and serve patients in homes or SNFs with flexible scheduling and potential for full-time roles.

Qualifikationen

• Master’s degree and NP licensure in state of practice.
• Active NP license with prescriptive authority.
• Board certification in a relevant adult-gerontology, family, or acute care specialty.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license and automobile insurance.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication.

Aufgaben

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other care team members.
• Coordinate with PCPs on patient education and follow-up as necessary.
• Provide services at the member’s residence, in homes or Skilled Nursing Facilities.
