Job summary

Matrix Medical Network provides in-home comprehensive health assessments across the nation, focusing on quality care for older adults and at-risk individuals. Providers conduct assessments, review medical histories, and coordinate with PCPs to close care gaps in a single visit.

This role offers flexibility in scheduling, mileage reimbursement, and opportunities for licensure support and continuing education while serving members in their homes or SNFs.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice to include prescription authority.
• Board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience preferred, new grads encouraged to apply!

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses and treatment, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and family members.
• Provide services at the member’s residence (home or Skilled Nursing Facility) and coordinate with PCP for follow-up.
