Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments for members, typically up to one hour per visit. You will review medical history, medications, and social factors to guide care and close gaps in health delivery.

You will collaborate with PCPs, case managers, and other clinicians, delivering care at homes or SNFs with a flexible, FT‑oriented schedule and state licensure support. Training and ongoing education are provided.

Qualifications

• Master’s Degree or commensurate NP experience.
• Current NP licensure in state of practice.
• Board certification in NP specialty.
• BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license.
• Automobile insurance coverage for state of residence.
• Strong computer skills.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, education, referrals, follow-up and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers as appropriate.
• Coordinate patient education and follow-up with PCP.
• Provide services at the member’s residence (home or Skilled Nursing Facilities).
