Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments, reviewing medical history, medications, and social factors during visits that last up to an hour. You will collaborate with physicians and care teams to close care gaps and support member well-being in their home or nearby facilities.

Qualifications

• Must have NP licensure in state of practice, with prescriptive authority as needed.
• AANP/AACN/ANCC board certification in Adult, FNP, AGPC, or related fields.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.

Responsibilities

• Perform adult/geriatric health assessments including medical history, diagnoses, treatment plans, and documentation.
• Administer point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers, social workers, and family members to coordinate care.
• Provide in-home visits at members' residences or SNFs as appropriate.
• Educate patients and caregivers to improve understanding and adherence.
