Job summary
Matrix Medical Network seeks nurse practitioners to perform comprehensive in-home health assessments. You will review medical history, medications, and social factors to identify care gaps, and collaborate with physicians and care teams to deliver personalized care.
Providers conduct visits in patients’ homes or SNFs, with flexible scheduling and potential FT opportunities. Applicants should have NP licensure, prescriptive authority, and relevant certifications, with prior NP experience
Qualifications
- NP licensure in the state of practice with prescriptive authority or the ability to obtain
- Current certifications in BLS/ACLS/CPR as required
- Master’s degree in nursing or related field; NP board certification preferred
- 1 year NP experience preferred; new grads welcome to apply
Responsibilities
- Conduct adult/geriatric health assessments including medical history and diagnoses
- Review patient records, health history, and medications to identify care gaps
- Coordinate with physicians, case managers, and other professionals
- Provide patient education and follow-up in home settings
- Document findings and care plans per Matrix guidelines
