Matrix Medical Network in the United States is seeking a Nurse Practitioner to perform Comprehensive Health Assessments in members' homes or Skilled Nursing Facilities. Each visit lasts up to one hour and reviews medical history, medications, and social factors to identify care gaps and guide treatment.

You will work with physicians, case managers, and staff, focusing on patient education and follow-up while enjoying flexible scheduling and a compensation model based on assessments completed.

Qualificações

• Master’s Degree in a relevant field or commensurate experience.
• Current NP licensure in state of practice with prescriptive authority as needed.
• Board certification in Adult/Adult Gerontology/Family/ACNP/ACMG/ Emergency or related field.

Responsabilidades

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, and referrals.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, social workers, and family members to coordinate care.
• Provide services at the member’s residence, including homes or skilled nursing facilities.
