Matrix Medical Network seeks nurse practitioners to perform comprehensive in-home health assessments. Each visit lasts up to an hour and covers medical history, adherence, and risk assessment to close care gaps.

You will collaborate with physicians and care teams to deliver personalized, high-quality care at the member’s residence. The role supports home or SNF visits, with ongoing onboarding, preceptorship, and continuing education.

Qualificações

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in the state of practice with prescriptive authority.
• AANP/AACN/ANCC board certification in Adult, Gerontology, Family, or Emergency.
• BLS/ACLS certification current.
• 1 year NP experience preferred; new grads welcome.
• Valid driver’s license and auto insurance for residence state.
• Strong computer skills and familiarity with health record software.
• Excellent verbal and written communication skills.

Responsabilidades

• Conduct adult/geriatric assessments including history, diagnosis, and education.
• Administer point-of-care and lab tests as needed.
• Coordinate with physicians, case managers, and caregivers for referrals.
• Collaborate with PCP on education and follow-up.
• Provide services at members' homes or SNFs.
