Job summary

Matrix Medical Network seeks licensed NPs to conduct Comprehensive Health Assessments in patients' homes or Skilled Nursing Facilities across the United States. During visits, you review history, medications, and social factors to identify and close care gaps in a single visit.

We offer flexible scheduling and competitive pay ($1,800–$11,500/month based on visits), mileage reimbursement, and 401(k). There is onboarding support, a preceptor program, licensure expansion support, and opportunities

Qualifications

• Master’s Degree or equivalent; NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• AANP, AACN, or ANCC board certification in applicable NP specialty.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome.

Responsibilities

• Perform adult/geriatric assessments including med history and diagnosis.
• Order and administer point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and others.
• Educate patients and follow up with PCP as needed.
• Provide services at the member’s home or SNF.

Education

Master’s Degree / NP licensure

NP licensure in practice state

Board certification (Adult / Gerontology / Family / Acute Care)

BLS/ACLS/CPR certified

Valid state driver’s license
