Job summary
Matrix Medical Network is actively seeking Nurse Practitioners to perform Comprehensive Health Assessments in members’ homes or Skilled Nursing Facilities. Each visit can last up to an hour and focuses on health history, medications, and risk factors to identify gaps in care.
Providers enjoy flexible scheduling, competitive pay based on completed assessments, mileage reimbursement, a 401(k) plan, and ongoing education support.
Qualifications
- Master’s Degree in Nursing and NP licensure.
- Current NP licensure in state of practice with prescriptive authority.
- Board certification in Adult, Adult Gerontology, Family or Emergency care.
- Current BLS, ACLS or CPR certification.
- 1 year NP experience preferred; new grads welcome.
- Valid state driver’s license and auto insurance coverage.
- Strong computer skills and EMR familiarity.
- Excellent verbal and written communication.
Responsibilities
- Perform adult/geriatric assessments including medical history, diagnosis and treatment, education, referrals and documentation.
- Administer and order point-of-care and lab-based testing as needed.
- Collaborate with PCPs, case managers, social workers and caregivers.
- Provide services at members’ homes or SNFs.
- Document according to Matrix guidelines and protocols.
