Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform in-home Comprehensive Health Assessments. Each 1-hour visit covers medical history, medications, adherence, and care gaps, with coordination from PCPs and care teams. Visits occur at members’ homes or SNFs nationwide.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in applicable NP specialty.
• Current BLS/ACLS/CPR.
• 1 year NP experience preferred, new grads welcome.

Responsibilities

• Conduct adult/geriatric comprehensive assessments during in-home visits.
• Review medical history, diagnoses, treatments, and medication adherence.
• Coordinate with PCPs, case managers, social workers, and care teams.
• Provide education and follow-up per Matrix guidelines.
• Document findings accurately in medical records.
