Job summary

Matrix Medical Network seeks nurse practitioners to perform comprehensive in-home health assessments. Each visit lasts up to one hour and includes review of medical history, medications, and social factors to identify gaps in care.

Providers administer point-of-care tests, coordinate with PCPs and care teams, and may work in homes or skilled nursing facilities. Flexible scheduling and competitive pay are offered.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• AANP, AACN, or ANCC board certification in Adult, Gerontology, Family or Acute Care nursing.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license.
• Auto insurance coverage as required by state.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments and document findings per Matrix guidelines.
• Order point-of-care and lab-based testing as needed.
• Collaborate with PCP, case managers, social workers, and caregivers.
• Educate patients and provide follow-up as necessary.
• Provide services at members' homes or Skilled Nursing Facilities.
