Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform in-home Comprehensive Health Assessments for older adults and at-risk individuals. Visits can last up to one hour and cover medical history, medications, social environment, and care gaps to improve health outcomes.

We offer flexible scheduling, with visits Sunday–Saturday 8am–7pm and opportunities to transition to full-time roles. Expect earnings from $1,800 to $11,500 per month depending on volume and state rates, plus education and

Qualifications

• Master’s Degree in Nursing or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• AANP/AACN/ANCC board certification in Adult or Family/Gerontology specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license.
• Auto liability coverage or ability to obtain.
• Strong computer skills and familiarity with medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric health assessments, including medical history, diagnosis, treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP for education and follow-up.
