Qualifikationen

• MSN required with NP licensure in CT.
• Active NP license in state of practice.
• Board certification: ANCC/AANP is preferred.
• Current BLS/ACLS certification.

• Conduct adult/geriatric assessments with medical history and documentation.
• Administer and order point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Provide services at members' residences (homes or SNFs).
