Matrix Medical Network offers in-home comprehensive health assessments across the nation. As a clinician, you will conduct geriatric assessments, review medical histories, and collaborate with a multidisciplinary team to close care gaps.

Qualifikationen

• Master’s degree or commensurate NP experience.
• Current NP licensure in state of practice with prescriptive authority.
• AANP, AACN, or ANCC board certification in appropriate NP specialty.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Aufgaben

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other care team members.
• Coordinate patient education and follow-up with PCP.
• Provide services at the member’s place of residence (home or Skilled Nursing Facility).
