Job summary

Matrix Medical Network seeks experienced nurse practitioners to conduct comprehensive in-home health assessments for older adults and at-risk patients. You will review medical history, medications, and social factors to identify gaps in care and improve outcomes.

Qualifications

• Master’s degree in a health-related field and NP licensure required.
• Board certification in Adult/Family/AGNP or related specialty preferred.
• Current BLS/ACLS or CPR certification preferred and active state license.

Responsibilities

• Perform adult/geriatric health assessments including medical history and documentation.
• Coordinate with physicians, case managers and staff to support care plans.
• Order and interpret point-of-care tests as needed.
• Provide patient education and follow-up to close care gaps.
• Deliver services at patients’ homes or skilled facilities as required.
